In order to be considered for an adoption you must:
  • 1. Be 18 years of age
  • 2. Have the knowledge and consent of all adults living in your household
  • 3. Have a valid ID with current address
  • 4. Understand that completing this application does not guarantee adoption and that the PVACI must approve your application.


  • Incomplete applications will not be processed.

    I certify that the information provided is true and accurate to the best of my knowledge and understand that falsification of information can be cause for denial of my adoption.


    By submitting this form, I agree to abide by all local animal ordinances, leases, and property ownership agreements as they pertain to the legally defined number of pets allowed in any human domicile or associated/attached properties.

    I further agree that, should I be in violation of any local animal ordinances, leases, and property ownership agreements pertaining to the number of animals allowed, I hold harmless Pet Versatility Rescue, Doggie KittieWorld4Us Rescue, and all associated Rescue organizations in all matters, and assume all legal and/or financial liabilities pertaining to any violations of said ordinances, leases, and/or agreements.

    I certify the above to be true and complete to the best of my knowledge. Typing your signature in this form is acting as your legal signature to this Adoption application. We reserve the right to deny any application.


    There is absolutely NO guarantee expressed or implied that any person submitting this application will be selected to adopt a dog. Please be prepared to correspond with the adoption coordinator via email. Due to restrictions on application volume, time, and the general nature of rescue, almost all of our correspondences are done by email. Please check your email regularly to see if you have been contacted.



    The process before you adopt:

  • Why do I want a companion?
  • Am I willing to commit to training? ( If under a year this will include some housebreaking )
  • Am I willing to spend time with my new companion?
  • Am I ready for the cost of vet care?


  • Consider the top 10 reasons pets are given up to shelters:

    1. Moving
    2. Landlord issues
    3. Cost of pet maintenance
    4. No time for pet
    5. Inadequate facilities
    6. Too many pets in home
    7. Pet illness(es)
    8. Personal problems
    9. Behavior problems
    10. No homes for litter mates

    How to prepare for the adoption process:

  • Provide us with 3 references
  • Provide us with a veterinarian reference
  • If applicable provide us with your landlords contact information


  • The Process After you fill out the adoption application:

  • You will be contacted by via mail on the status of your application.
  • A pre-adoption phone interview will be setup with an Adoption Specialist
  • A home check may be scheduled
  • You will be contacted on your application status


  • Adoption Companion fees: Varies

    If approved for adoption:

  • A adoption date is set
  • An adoption contract is signed


  • For companions adopted under 6 months:

    State law requires that the adopter sign a contract that he/she will get the animal spayed or neutered by the time it reaches 6 months old. Pet Versatility holds a deposit of $90 that is refunded when proof of spay/neuter is provided by your veterinarian.

    Holds: If you are considering holding your desired pet please read the following.

  • We require a deposit which goes toward the adoption of your pet.
  • We will hold the animal for you while we process your application.
  • If you are not approved to adopt your deposit is refunded.
  • If you change your mind your deposit is non-refundable.


  • Why: We want you to be certain that this is a decision you have thought through thoroughly & not just a whim so if you change your mind for any reason, your deposit is not refundable. When potential adopters change their mind after the puppy has been held for them, their chances of getting a good home is passed up because you made a promise to provide the animal with a home. These matters make it harder on the animal that was promised a home.

    ****If you rent, please let your landlord or management company know we will call as we will want confirmation that your landlord or management company agrees to your fostering dogs on their property.


    This form will be reviewed, prior to adoption approval of any animal you are waiting to adopt. An interview is conducted with every potential adoptive family to determine that adoption criteria have been met and to carefully match a dog with a new and permanent home. A home visit evaluation may be required prior to your application being for adoption.

    Application Fee: A $25.00 application fee is required to process your application to help defray postage, copying and phone costs. Application fees are nonrefundable and do not guarantee approval.
